Chronic Disease Self-Management Program

Become a volunteer leader for Chronic Disease Self-Management Program. The goal of the evidence-based Chronic Disease Self-Management and Diabetes Self Management Programs is to give individuals with chronic conditions the opportunity to learn how to manage and improve their own health while reducing health care cost.

As a leader you will:

  • Complete 5 days of training to become certified in CDSMP and DSMP
  • Co-facilitate small groups with another leader
  • Lead one workshop within one year of certification
  • Change Lives

This program emphasizes the skills needed in day-to-day management of treatment and to maintain or increase activity:

  • Offered to participants age 60+ with chronic conditions and family members
  • Classes take place once a week for six weeks
  • Classes last 2.5 hours per week
  • Classes offered at no cost to participants
  • Participants learn to: manage symptoms, incorporate exercise, monitor nutrition, make informed treatment decisions and deal with emotions
  • Transformative techniques to drastically improve health outcomes
